Why Autonomous Server AI Fails in Production — and How Our Approval State Machine Works

An AI loop that executes shell commands on production servers without asking is an operational hazard. Below is the visual timeline of the 45-second staging incident that ended our early uninhibited prototype — and the deterministic Approval State Machine that governs RasnuOS today.

Chart 01 · Incident Telemetry Reconstruction (8 GB Staging Database Node)

Anatomy of an Uninhibited 45-Second AI Feedback Loop

Why Blind Autonomy Fails
T + 00sRAM: 5.1 / 8.0 GB
1. Local Optimization

Load test triggers slow MySQL queries. Uninhibited prototype raises `innodb_buffer_pool_size` from `4 GB` to `7.5 GB` on an `8 GB` host.

T + 18sRAM: 7.98 / 8.0 GB
2. Kernel OOM-Kill

Host memory saturates completely. Linux kernel OOM-killer wakes up and terminates the primary `mysqld` daemon.

T + 24sSTATUS: CRASH LOOP
3. Recovery Deadlock

AI sees `mysqld` down, fires `systemctl restart mysql`, and hits an InnoDB crash-recovery timeout under the oversized config.

T + 45sSTATUS: HOST REBOOT
4. Compounding Fault

Assuming a stale kernel lock is holding the volume, the uninhibited loop issues a full host reboot while InnoDB is mid-recovery.

Where RasnuOS Intercepts Today (T + 03s): Read-only triage spots the slow query AND available host RAM (`8 GB`), stages a safe config diff, and pauses on an Approval Card before any memory setting is mutated.

Flowchart 02 · Deterministic Execution State Machine

How Every Action Transitions From Triage to Audited Execution

Enforced in Control Plane Persistence
STATE 01 · AUTOMATIC
READ_ONLY_TRIAGE

Inspects live telemetry, service status, and log tails. State-changing operations cannot execute in this state.

STATE 02 · POLICY SCORING
SAFETY_CLASSIFICATION

Evaluates obfuscation filter, semantic blast radius (`Low`, `Medium`, `High`, `Critical`), and multi-OS RBAC rules.

STATE 03 · EXECUTION PAUSED
AWAITING_SIGN_OFF

Persists exact shell payload, unified file diff, and rollback command. Awaits human or change-board sign-off.

Branch A · Console Approval Card

Authorized `Operator`, `Admin`, or `Owner` clicks Approve & Execute directly inside the workspace.

Branch B · External Team / CAB Gate

Dispatches outbound approval request to ServiceNow, Telegram, Email, or Custom Webhook for team sign-off.

Branch C · Policy Reject / Cancel

Destructive or rejected proposals transition immediately to `TERMINATED` with zero packets sent to the server agent.

STATE 04 · VERIFIED EXECUTION & 1-CLICK UNDO
Agent verifies compiled local OS protection rules, saves `.bak` file snapshot, runs inside a bounded process group, and appends the output + approver ID to the SHA-256 tamper-evident audit ledger.

Who Can Inspect, Approve, and Govern Production Changes

Operational Capability Viewer Operator Admin Owner
View 15s Telemetry, Metrics & Read-Only Diagnostics Allowed Allowed Allowed Allowed
Approve Standard Service Restarts & Config Diffs Read-Only Allowed Allowed Allowed
Manage Encrypted Credential Vault & Custom Blocklists Denied Scoped Use Full Access Full Access
Configure External Approval Webhooks & Export Audit CSV Denied Denied Allowed Allowed
Organization Billing, Plan Upgrades & Server Revocation Denied Denied Allowed Owner Authority
ServiceNow & Change Boards: High-impact operations can automatically create a Change Request and pause execution until the external webhook returns an approved status.
Telegram & Email Alerts: On-call leads receive real-time notifications with the target server, proposed command, and risk classification as soon as an Approval Card is staged.
